Report of Assessments have numbers 0,1,2,3 etc

When admins download responses from assessments, they get 0,1,2,3 in the csv file as option selected by the user

We are fixing this bug in our next release

But here is a breakdown of what the numbers mean:

• 0 → Option 1

• 1 → Option 2

• 2 → Option 3

• 3 → Option 4
